Zoryve Foam Approved for Scalp and Body Psoriasis: A Steroid-Free Topical Option

Medically reviewed by Dr. Rimti, MBBS

The FDA approved Zoryve (roflumilast) foam 0.3% in May 2025 for plaque psoriasis on the scalp and body in adults and adolescents aged 12 and older. This once-daily, steroid-free foam cleared scalp psoriasis in two-thirds of patients and provided itch relief within 24 hours of the first application. Unlike topical steroids, it has no time limits on use.

Scalp psoriasis is one of the most frustrating forms of the condition to manage. About half of all people with plaque psoriasis deal with scalp involvement, and the treatment options have long been limited.

Topical steroids work but can’t be used indefinitely because of skin thinning. Coal tar shampoos are messy. And getting thick creams through hair to reach the scalp is a challenge in itself.

Zoryve foam tackles several of these problems at once. The clinical data behind it is strong enough to make it a real option in your treatment plan.

How Does Zoryve Foam Work?

Zoryve foam contains roflumilast, a PDE4 inhibitor that reduces inflammation by blocking an enzyme called phosphodiesterase 4 inside immune cells. By preventing PDE4 from breaking down a chemical called cAMP, roflumilast quiets the inflammatory cascade that drives psoriasis plaques. It’s steroid-free, so it doesn’t carry the skin-thinning risks of topical corticosteroids.

PDE4 inhibitors work differently from steroids. Steroids broadly suppress the immune response where you apply them. This works, but it comes with consequences over time. PDE4 inhibitors target a more specific step in the inflammation process.

You may already know Otezla (apremilast). It’s a PDE4 inhibitor pill used for moderate-to-severe psoriasis. Zoryve works through the same pathway but goes directly on your skin. This means the drug stays where you need it rather than moving through your whole body.

The foam formula is made for areas with hair. Unlike creams or ointments that feel greasy and sit on top of hair, foam spreads more evenly across the scalp. It absorbs without leaving heavy residue.

What Did the Clinical Trials Show?

In the ARRECTOR Phase 3 trial, 66.4% of patients with scalp psoriasis achieved clear or almost clear skin (S-IGA success) with Zoryve foam, compared to 27.8% using the vehicle (inactive foam). For body psoriasis, 45.5% achieved clear or almost clear skin vs. 20.1% with vehicle. Itch relief began within 24 hours of the first application.

The ARRECTOR trial results stand out for several reasons:

Scalp results:

  • 66.4% achieved S-IGA success (clear or almost clear scalp) vs. 27.8% vehicle
  • That’s a 38.6 percentage point difference over the inactive foam

Body results:

  • 45.5% achieved B-IGA success (clear or almost clear body skin) vs. 20.1% vehicle
  • That’s a 25.4 percentage point difference

Itch relief:

  • Patients reported reduced itch within 24 hours of their first application
  • Itch is consistently rated as the most bothersome psoriasis symptom by patients, so fast relief matters

These numbers compare favorably to other topical psoriasis treatments. While the scalp clearance rate of 66.4% is slightly below what potent topical steroids achieve, the advantage is that Zoryve can be used continuously without the worry of steroid-related side effects.

Why Does Steroid-Free Matter?

Topical steroids are effective for psoriasis but can’t be used long-term, especially on the scalp and face, because they thin the skin over time. Steroid-free treatments like Zoryve can be applied with no time limits. This gives people with chronic scalp psoriasis an ongoing option without the risks of long-term steroid use.

If you’ve managed your psoriasis for any length of time, you’ve likely heard your dermatologist say something like “use this for two weeks, then take a break.” That on-off cycling is standard with topical steroids because extended use can cause:

  • Skin thinning (atrophy)
  • Stretch marks
  • Broken blood vessels
  • Rebound flares when you stop

These risks are higher on thinner skin areas like the scalp, face, and skin folds. For a chronic condition like psoriasis, a treatment you can use nonstop without these worries fills a real gap.

Zoryve foam doesn’t have a recommended maximum treatment duration. In the ARRECTOR trial, people used it daily without the skin damage linked to long-term steroid use. That gives you and your skin doctor more freedom to plan your long-term care.

How Do You Use Zoryve Foam?

Apply Zoryve foam once daily by shaking the can, applying a thin layer to affected scalp or body areas, and letting it absorb. You don’t need to cover the area with bandages. The foam absorbs quickly without heavy residue, making it practical for use on hair-bearing areas.

The application process is straightforward:

  1. Shake the can before each use
  2. Apply a thin layer to the affected areas on your scalp or body once daily
  3. Avoid covering the treated area with airtight bandages

You can use Zoryve on your scalp, trunk, and limbs. It absorbs fairly fast and doesn’t leave the heavy residue that some ointments do. This matters for scalp psoriasis, where greasiness can make hair care harder.

Once-daily use is simpler than many topical routines that require twice-daily application. Fewer steps usually means people stick with it better, which leads to better results.

Who Is a Good Candidate for Zoryve Foam?

Zoryve foam may be a good fit if you have scalp psoriasis that hasn’t responded to medicated shampoos, if you’ve been cycling on and off topical steroids, or if you have psoriasis in areas where steroid use carries higher risk. It’s approved for adults and adolescents aged 12 and older and can also be used alongside systemic treatments for stubborn spots.

Zoryve foam may be a good fit if:

  • You have scalp psoriasis that hasn’t responded well to medicated shampoos
  • You’ve been cycling on and off topical steroids and want a long-term alternative
  • You have psoriasis in areas where steroid use carries higher risk (scalp, face, skin folds)
  • Your psoriasis is mild to moderate and you prefer topical treatment over systemic medications
  • You’re an adolescent aged 12 or older (Zoryve foam is approved for this age group)

For people with more extensive or severe psoriasis, Zoryve foam can also work as part of a combination approach alongside systemic treatments or biologics. Some people use a biologic for overall disease control and a targeted topical for stubborn spots that don’t fully clear.

What About Zoryve Cream?

Zoryve cream (roflumilast cream 0.3%) was approved earlier for plaque psoriasis on the body. The newer foam version was made to make applying the drug to the scalp and hairy body areas easier. The FDA is also reviewing roflumilast cream for children ages 2-5, with a decision expected by June 29, 2026.

Zoryve already existed as a cream formulation (roflumilast cream 0.3%), approved earlier for plaque psoriasis on the body. The foam version was designed to make applying the drug to the scalp and hairy areas easier. If you’ve tried the cream and found it difficult to use on your scalp, the foam may solve that problem.

The FDA is also reviewing roflumilast cream for children ages 2-5, with a decision expected by June 29, 2026. If approved, it would give very young children with psoriasis a steroid-free topical option. This matters because children’s skin is more prone to steroid side effects.

What Questions Should You Ask Your Dermatologist?

Ask whether Zoryve foam can replace or complement your current scalp treatment, whether it’s appropriate for sensitive areas like the face or skin folds, what your insurance will cover (including manufacturer copay cards from Arcutis), and whether daily or flare-only use is right for your pattern.

  1. Could Zoryve foam replace my current scalp treatment? Ask whether it’s appropriate as a standalone treatment or an add-on to what you’re already using.
  2. Can I use it on my face or skin folds? The approval is for scalp and body. Ask your doctor about off-label use on sensitive areas.
  3. What will my insurance cover? Newer branded topicals can be expensive. Ask about manufacturer copay cards and patient assistance programs from Arcutis.
  4. Should I use it daily or just during flares? Some people benefit from continuous use; others use it as needed. Your dermatologist can guide this based on your pattern.
